When scheduling appointments, we ask for your help in maintaining the excellence in eye care that you deserve. We will try and provide a time that best fits your schedule. If you find that you cannot keep your appointment, kindly give our office a 48 hour notice to avoid a missed appointment fee.

If you are a new patient, you can also print out the Vision History Form [pdf] and bring it with you to the office, saving you from having to fill it out when you arrive.

Insurance

Our office files insurance with several plans as participating providers. When you call our office please state your specific plan. If we are not participating with your particular insurance plan chances are we will be able to file the services, as non-participating providers.

While we file your insurance as a courtesy, we cannot guarantee what your benefits will pay, so we encourage you to contact your insurance company for your benefit information.
